Some universities and courses require an additional form alongside your UCAS application. Its questions, deadline and submission route belong to that provider or course, so submitting UCAS does not necessarily complete every part of the application.
A supplementary application form is an extra part of a particular university or course application that you complete outside the standard UCAS form. There is no single supplementary form used across UK universities.
For courses using the UCAS undergraduate route, you still submit the main application through UCAS. How to Complete Your UCAS Application covers that process. A university then collects any extra information it requires through the route it specifies, such as its own form, questionnaire or applicant portal.
Some universities publish the requirement on the course page before you apply. Liverpool's 2027 Veterinary Science guidance, for example, tells every applicant to submit work-experience evidence through a separate online form. Others introduce the extra step after receiving UCAS. Cambridge emails most undergraduate applicants a link to My Cambridge Application, while Surrey sends its 2027 Veterinary Medicine questionnaire after the initial application screening stage.

The university sets the deadline for its additional form. Meeting the UCAS deadline does not satisfy a separate deadline attached to one of your choices.
For 2027 entry, most Cambridge applicants must submit UCAS by 15 October 2026 and My Cambridge Application by 6pm on 22 October. Oxford's 2027 Graduate Entry Medicine applicants also have a 22 October deadline for the Oxford Supplementary Application Form, including two additional references. Liverpool Veterinary Science gives UCAS applicants until 29 October to submit its separate work-experience evidence.
If a course publishes its supplementary requirement in advance, prepare the information or evidence before you send UCAS. Oxford specifically advises Graduate Entry Medicine applicants to request their two additional references early enough for the referees to respond before the supplementary deadline.
If you apply through a school or college, include its internal UCAS deadline in your planning. Pressing Send on a linked application sends it to your centre first, not directly to UCAS. The centre still needs time to check the application, add the reference and submit it to UCAS. For a course that releases its supplementary form only after receiving the UCAS application, leaving the school submission until the last possible point leaves less time available for the next step.
If you are applying to several courses with extra selection requirements, track each choice separately. A completed UCAS application does not tell you whether every university has finished collecting what it needs from you. Keep one record of outstanding forms, questionnaires, tests, portfolios, auditions and deadlines for each choice.
Supplementary forms collect information the provider needs for that course or applicant, such as course-specific experience, more detail about studies or qualifications, additional references, supporting documents or information used in its selection process.
Cambridge's My Cambridge Application, for example, asks different questions according to the applicant's circumstances, including information about topics studied and transcripts where required. Liverpool uses its separate submission for veterinary work-experience evidence. Oxford Graduate Entry Medicine uses its form to collect two referees who are additional to the referee on the UCAS application.
Follow the instructions for the form you have actually been asked to complete. If the university requests a document through its portal or form, use that route. Uploading or emailing the same document elsewhere does not satisfy the requirement unless the provider says it does.
For 2027 entry, UCAS allows applicants to upload certain supporting documents within the main application. UCAS says those uploads are optional and universities still contact applicants directly when they need additional information. A provider's own instructions continue to apply. Cambridge states that it will not download or review additional documents uploaded to UCAS and tells applicants to supply documents it requires through My Cambridge Application or the relevant College route.
Names, qualifications, dates and other factual details on the supplementary form should agree with your UCAS application unless something has genuinely changed or the university is deliberately asking for information that UCAS did not collect.

Answer supplementary questions with the additional information requested. Cambridge's syllabus questions and the additional referees required by Oxford Graduate Entry Medicine are examples of information deliberately collected outside the standard UCAS application.
Contact the admissions team before the deadline if you cannot tell whether the requirement applies to you, what a question is asking, which document is required or where to submit it. Include your course and UCAS Personal ID to identify the application, and quote the instruction you need clarified.
When a university says it will send a form after receiving your application, check your spam folder and applicant portal if the message has not arrived within the period it gives you. If nothing appears, contact admissions while there is still time to resolve the problem before the deadline.
The consequences of missing a supplementary deadline depend on the course. Oxford states that late or incomplete Graduate Entry Medicine applications will not be considered. Liverpool states that a Veterinary Science application without its separate work-experience submission is incomplete and will be rejected. For those courses, submitting UCAS on time does not make an application complete if the required provider form is missing.
If you have already missed a deadline and the university has not stated what happens next, contact admissions and ask whether the form is still open and whether your application remains under consideration. The answer depends on that university's course rules.
After submitting the form, keep its confirmation or a copy of what you sent where the system allows this, and continue checking the communication channels the university has told you to use until it makes its admissions decision.
